Tricare East Humana Military
I am an authorized out-of-network provider for TRICARE. TRICARE covers 6 one-hour consults. I will bill TRICARE on your behalf.
If you’re enrolled in TRICARE Prime or are Active duty, you will need a referral from your primary care physician or manager if you see a non-network provider. Point-of-service charges may apply without the referral.
TRICARE Select does not need a referral.
$25 cancellation fee (not covered by insurance) if not canceled 24 hours before the appointment.

Self-Pay
If you are not covered by Tricare or your insurance is not billable by TLN you may self-pay for services.

HSA (Health Savings Accounts) can be utilized as well as Cash, Check, and Credit Cards. Payment is expected at the end of the visit. I cannot guarantee that you will be reimbursed. Please contact your insurance provider for guidance.
